After a year of construction staking calcs I've seen a lot of misplaced or lost construction staking requests. How do you receive requests?

1. Phone
2. Fax
3. Email
4. Web Form
5. Other

Taking a web programming class at the local community college encouraged me to take a look at a web based request system that stores project, request and crew reports in a database. And since I'm now one of the "unemployed" I've had time to work on it and I'd appreciate any suggestions or comments.
